Guitar - Beginner - Ages 8-12 - Mondays 4:00PM-4:40PM - 9 class weeks - March 31st thru June 9th (no 5/12 - recitals week, no 5/26 - Memorial Day)

Guitar - Intermediate - Ages 10-14 - Mondays 4:45PM-5:25PM - 9 class weeks - March 31st thru June 9th (no 5/12 - recitals week, no 5/26 - Memorial Day)

Guitar - Adult Beginner - Ages 15+ - Wednesdays 6:30PM-7:10PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Guitar - Adult Intermediate - Ages 15+ - Wednesdays 5:45PM-6:25PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Ukulele - Beginner - Ages 6-10 - Wednesdays 2:45PM-3:25PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Ukulele - Intermediate - Ages 8-12 - Wednesdays 3:30PM-4:10PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Voice - Beginner - Ages 8-12 - Wednesdays 4:15PM-4:55PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Voice - Intermediate - Ages 10-14 - Wednesdays 5:00PM-5:40PM - 10 class weeks - April 2nd thru June 11th (no 5/14 - recitals week)

Notes:

We may consider ages outside of the range listed, based on a discussion with the student/parent. Please note this request in the “additional info” on form if applicable.
Recitals Week (applies to fall and spring sessions) - Group students will be invited and encouraged to perform in our full-program recitals (more info to come)
Final Class Day - Group students will work towards a special performance for family on the final class day.

Release of Liability Waiver

As the student, or parent/guardian of my student(s) enrolling in music lessons, I understand that there may be unforeseen risks of personal injury or property damage. I am voluntarily allowing myself and/or my student(s) to assume such risks. I hereby agree to waive all liability and hold harmless Music Flow and/or agents for any injury or damage that may occur to my child while my student(s) is participating in any Music Flow lessons, recitals, or special events.

I fully understand and agree to this release of liability.
